Job Title: RN Case Manager

Location: Tuba City, AZ

Job Type: Contract

Duration: 13 Weeks

Shift: Monday to Friday, 8:00 AM – 5:00 PM with Weekend Rotation and Call

Position Overview:

Pride Health is seeking a skilled RN Case Manager to join the team. This contract position is ideal for a dedicated professional with experience in case management and clinical care. The role involves managing high-risk patients, providing a seamless continuum of care, and working with a multidisciplinary team to improve patient outcomes while ensuring resource optimization.

Key Responsibilities:

• Supervise and support high-risk patients.
• Design and manage a continuum of care for clients.
• Perform assessments, service planning, and resource acquisition.
• Link clients with community service providers.
• Ensure effective use of healthcare resources through utilization review.
• Work with a multidisciplinary team to reduce care fragmentation and improve patient outcomes.

Qualifications:

• Associate’s degree in Nursing (must obtain a Bachelor's degree within 2 years)
• Current Basic Life Support (BLS) certification (American Heart Association)
• Valid RN license (any U.S. state, Puerto Rico, or U.S. territory)
• Minimum of one year of clinical care experience, with knowledge of discharge planning, case management, or utilization review
• Familiarity with electronic health record systems
