Job Description

Key Responsibilities

Member Support & Navigation

  • Build trusting relationships with members and meet them “where they are.”
  • Assess oral health needs, social determinants of health, and barriers to care.
  • Support members in following provider treatment plans and home care instructions.
  • Assist members in locating dental providers, scheduling appointments, and arranging transportation or translation services.
  • Help members understand available dental benefits.
  • Address barriers such as dental phobia, behavioral health concerns, or lack of knowledge about oral health.
  • Provide in-person support when appropriate.
  • Collaborate with referral sources (health systems, state agencies, community organizations) to co-manage oral, medical, and behavioral health needs.
  • Refer members to social services to reduce external barriers to care.
  • Document all actions in the care coordination platform according to program standards.

  • Participate in case consults, escalations, and case conferences.
  • Contribute to teamwork, continuous feedback, and solutions that improve processes and outcomes.
  • Commit to ongoing learning and professional development.

Qualifications

  • Bachelor’s degree in social work, human services, community health, or a related field (preferred).
  • Experience in care coordination, case management, or community health navigation (health systems, Medicaid programs, or behavioral health preferred).
  • Experience supporting populations facing barriers to care.
  • Bilingual skills strongly preferred.
