About the Role

In this role, you will provide social services support, counseling, and advocacy to individuals with complex needs. You’ll collaborate closely with an interdisciplinary healthcare team to coordinate care, address barriers, and connect members with the resources they need to thrive.

Responsibilities

• Address and manage the social service needs of One Care members.
• Conduct follow-up sessions with clients in-person or by phone.
• Provide brief counseling, advocacy, and community resource referrals.
• Participate in interdisciplinary team case conferences.
• Conduct home visits independently or with team members in the community.
• Serve as an active, integral member of the primary care/behavioral health team.
• Advocate for patients with outside agencies, such as schools or social service organizations.
• Provide crisis intervention and support as needed.

Qualifications

• Must Have: Bachelor’s Degree in Social Work, Human Services, or Psychology.
• Nice to Have: Bilingual in Spanish.
• Experience working with both children and adults.
• Background in dual diagnosis (mental health/substance use).
