About the Role...

Job Details/Quick Notes:

  • Length: 13 weeks with extension opportunity or perm!
  • Schedule: Monday - Friday; NO major Holidays, and NO on-call
  • Shift: 5x8's Days

Travel vs. Local:

  • Travel Contract which requires the LMSW, LGSW, or LCSW to live 51 miles outside of "Location" (see above) to qualify for Stipend

Travel Pay Package:

  • Untaxed Travel Stipend: $1,600
  • Weekly Gross: $2,200 - $2,280

Skill Required:

  • Discharge Planner Case Manager
  • Shifts: 800-1630 (8A-430P) 5 x 8 Days
  • On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
  • Floating Requirement: no floating requirement
  • Local Rate within 60 miles of facility

Required Experience/Licensure:

  • Will accept RN Case Manager Discharge Planner or Licensed Social Worker - Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already*
  • - DC RN Licensure - ** WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION* (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW
  • - BLS
  • - 2 years of experience
  • - Must have acute hospital discharge planning experience
  • - Skills required: Emergency Department, Case management
