About the Role...

Length: 13 weeks with extension opportunity or perm!
Schedule: Monday - Friday; NO major Holidays, and NO on-call
Shift: 5x8's Days

Travel vs. Local:

Travel Contract which requires the LMSW, LGSW, or LCSW to live 51 miles outside of "Location" (see above) to qualify for Stipend

Travel Pay Package:

Untaxed Travel Stipend: $1,600
Weekly Gross: $2,200 - $2,280

Skill Required:

Discharge Planner Case Manager
Shifts: 800-1630 (8A-430P) 5 x 8 Days
On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
Floating Requirement: no floating requirement
Local Rate within 60 miles of facility

Required Experience/Licensure:

Will accept RN Case Manager Discharge Planner or Licensed Social Worker - Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already*
- DC RN Licensure - ** WILL ACCEPT PENDING DC LINCEUSNRE WITH PROOF OF APPLICATION* (Aya paid open license project) OR candidate can hold LMSW/LGSW/LCSW
- BLS
- 2 years of experience
- Must have acute hospital discharge planning experience
- Skills required: Emergency Department, Case management
