Physical Therapist Needed

Responsibilities
Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
Prepare and submit clinical and progress summaries based on identified and attained goals
Ensure that other treating medical professionals have the necessary information to evaluate the patient appropriately
Requirements
Must hold a Doctorate in Physical Therapy OR a Bachelor's degree in Physical Therapy and have passed the NPTE
Must have graduated from an accredited school.
Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
Current CPR certification.
