We are looking for a Florida-Licensed RN or LPN who is passionate about healthcare and addressing preventable health deterioration resulting from chronic illness. Responsibilities

• Executing remote chronic care mgmt of patients with chronic heart, kidney, and lung conditions.
• Setting up and maintaining care plans
• Executing patient medication reconciliation.
• Physiologic data monitoring and analysis
• Recruiting & Onboarding patients at rotating physician sites
• Interacting with patients to ensure compliance with care plans.
• Working together with MAs and other physiologic monitoring staff to deliver whole-person care.
• Developing product documentation and communicating with Vironix customers (clinics, hospitals, and individual providers).
• Developing deep expertise on the mobile/web software and hardware associated with Vironix patient monitoring technology.
• Developing powerpoint presentations and participating in internal/customer meetings.

Qualifications

• Must have a Florida certification as a RN or LPN
• Must be able to execute chronic care mgmt and remote physiologic monitoring activities through the Vironix web app software.
• Must have experience with and interest in direct patient interactions and facilitating communication between patients and medical specialists
• Must have strong computer skills (microsoft office, google docs/sheets/slides, web browsing, and familiarity with web/mobile applications)
• Must have experience in administering, gathering, and reviewing data.
• Must have Strong writing and communication skills.
• Must take responsibility for clinical monitoring and be comfortable as an individual contributor (this is a job where hand holding will not happen).
• Should have the ability to work in a fast-paced startup and rapidly innovate continuously.
