Sanitas is hiring a Full-Time RN Care Educator for our Memphis - Wolfchase facility in Cordova, TN. This Registered Nurse (RN) will perform patient health education through a unique care program, enabling patients to achieve self-management. Collaborating with a Care Promoter Medical Assistant (MA) and under the guidance of the Lead Care Program Nurse, the Care Educator will plan, coordinate, and monitor patients' needs to positively impact their chronic health conditions.

RN Care Educator Responsibilities:

• Identify patients eligible for the care program.
• Conduct direct and group patient education.
• Review health risk assessments to formulate care plans.
• Comply with documentation requirements and re-evaluate plans.
• Enter patient health information data.
• Follow up and monitor patients based on health risk status.
• Promote self-management and behavior modification.
• Educate patients and families about community resources.
• Collaborate with the healthcare team regarding patient care.
• Maintain a safe and aseptic environment.
• Supervise vaccine and clinical supply inventory.
• Assist in company health fairs and events.
• Local travel may be required to cover other facilities.

Required Skills & Qualifications:

• BS or graduate from an accredited nursing school required.
• IV Certification and active State of Tennessee RN License required.
• Proficiency in Microsoft Office required.
• Case Management experience preferred.
• CPR-BLS certification required; ACLS/PALS preferred.
• 2 years of care management or patient education experience preferred.

Physical Demands:
Regularly required to stand, walk, and lift up to 50 pounds, with specific vision abilities and exposure to various environmental conditions.
