Make a Difference in Pediatric Dialysis Care

Are you a dedicated LMSW or LCSW looking to make a meaningful impact in the lives of pediatric patients undergoing dialysis? We are seeking a hospital-based Pediatric Renal Social Worker to provide critical support, resources, and care coordination to children and their families as they navigate treatment and discharge planning.

Location: Baltimore, MD

Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Salary: $70,000 - $85,000 (Based on Experience & Licensure)

Employment Type: Full-Time

Key Responsibilities:

• Conduct patient assessments and coordinate essential resources for smooth discharge planning.
• Provide emotional support and education to patients and families coping with chronic kidney disease.
• Work in-hospital, meeting patients in their rooms and occasionally conducting virtual follow-ups post-discharge.
• Participate in interdisciplinary rounds with providers, utilization review, social work, and case management teams.
• Advocate for patient needs, ensuring access to appropriate community, financial, and medical resources.

What We’re Looking For:

Required Qualifications:

• Active LMSW or LCSW licensure (Maryland)
• Experience in Dialysis or Oncology Social Work
• Familiarity with Baltimore-area resources for pediatric and renal patients
• Open to new grads with relevant dialysis internship experience

Preferred Experience:

• Previous work in pediatric healthcare settings
• Experience managing high patient volumes in a hospital environment
