Job Description

Care Coordinator

Overview:

We are seeking a Care Coordinator to connect clients with essential services and resources in the community. This role involves managing client referrals, intake, scheduling, and ensuring seamless coordination of care between clients, families, providers, and community partners.

Key Responsibilities:

  • Facilitate client intake, referrals, and service coordination for mental and physical health needs.
  • Support client and family engagement to ensure satisfaction with program services.
  • Collaborate with providers and community organizations for comprehensive care.
  • Provide backup support for administrative staff as needed.

Qualifications:

  • Associate's degree in social services, behavioral health, or a related field (Bachelor's preferred).
  • Minimum 2 years' experience in social services, mental health, or medical care coordination.
  • Experience in clinical settings and underserved communities preferred.
  • LGSW certification is a plus but not required with relevant experience.
  • Background check required.

Details:

  • Pay: $24-$27 per hour (DOE)
  • Schedule: Mon-Fri, 8 AM - 5 PM
  • Location: Onsite in St. Paul
